Medical Examination Report (current within six months).
Immunization Record (current within six months).
Psychological Report (current within one year).
Social History (current within one year).
Dental Examination Report(current within six months)
Current IEP
Therapy Evaluations (Physical, Occupational, and speech if receiving).
Discharge Summaries from institutions (if applicable)
